Our Social Workers provide support across Australia as an online service.
We have strong connections in our local communities in Maitland NSW, Newcastle NSW, Gold Coast, QLD
and across all states and territories.
Individual Support
Our Social Workers focus on you as the individual. We develop meaningful support plans that precisley map the systems that support you, allowing us to truly understand every aspect of your needs.
Crisis Intervention
When life throws unexpected challenges, our Social Workers will support you to navigate this time. Crisis Intervention includes disruption to family units, health concerns, breakdowns in relationships amongst others.
Family Support
Whilst disability may belong to the individual there is an impact on the entire family unit. We focus on supporting all systems involved, bring a level of calmness and control to complex and difficulty situations.
School Support
We support the child or adolescent when issues arise and collaborate with the school in upholding the Disability Standards for Education and the CRPD.
Support & Specialist Support Coordination
Our Social Workers also assist with everything related to your plan. We can help you understand the plan, make referrals and collaborate with all in your support network.
Beyond School Support
Transitioning to life beyond school can be filled with excitement and apprehension. Our Social Workers will work with you to understand how you want your life to look, your goals, dreams, strengths and worries.
Capacity Building Therapy
We will support you and those involved in caring for you to understand your needs. We build detailed plans to make sure you are always building capacity and social functioning. No matter what goal, big or small, you are at the centre.
Functional Capacity Assessments
Our Social Workers also conduct FCAs. The NDIS recognise Social Workers as one of the professions with expertise in understanding, assessing and reporting on an individual’s functioning.
Support Worker Engagement Plans
We work alongside your support workers in developing and directing each individual’s support. Using SMART criteria, our role is to make sure that your support worker sessions are more than ‘hanging out’ but have measurable outcomes that build capacity.
SIL & ILO Support
Our Social Workers & Support Coordinators have extensive experience in supporting young people and adults gain independence through living in a home that fits their needs and lifestyle. They can support you to navigate and manage those complexities.
